What's everyone's mix ratio?

I'm trying to get my urologist on the same page with me, so would it be possible for several people to post their mix ratios, along with the specifics of what each one is? For example my current mix is Trimix #9 (papa/phent/pge1)(ds)30/2/0.02 mg/ml. Exactly as printed on my label. Just trying to get a better understanding for what everyone else is using
