Why does my progress keep getting deleted?

Whenever I finish chapter 1 and choose to continue playing on chapter 2, my progress from chapter 1 is basically erased and I lose my items (iron shackle, spooky sword, etc) and the random interactions in the town (Onion-san, sans, etc)

How do I fix this?
